Definition – abnormally low blood glucose level. A syndrome characterized by symptoms of sympathetic nervous system stimulation or of CNS dysfunction that are provoked by an abnormally low plasma glucose level.

  • Drug-induced (most common)
    insulin, alcohol, sulfonylurea’s account for 50% of hospitalized cases
  • Non-drug induced
    fasting hypoglycemia
  • CNS manifestations, usually during fasting or exercising reactive hypoglycemia
  • Occur only when provoked by a meal
  • Associated with less marked and briefer decreases in plasma glucose

Predisposing factors

  • Over consumption of sugar and processed foods
  • Inadequate consumption of complex carbohydrates (rice, whole wheat)
  • Inadequate protein consumption and digestion
  • Caffeine, alcoholism, cigarette smoking, drugs (injection of insulin)
  • Caffeine
  • High stress lifestyle
  • Frequently missed or delayed meals
  • Over exercising
  • Adrenal stress
  • GI surgery
  • Inherited defects –
  • Liver enzyme deficiencies that restrict liver glucose release
  • Defects in fatty acid oxidation (carnitine deficiency)
  • Extensive liver disease
  • Auto-immune hypoglycemia

Symptoms

  •  If meals are delayed or missed symptoms can range from:  fatigue, sleepiness, headache, depression, anxiety, nervousness, hyperactivity, irritability, crying for no apparent reason, mood swings, unexplained feelings of insecurity or fear, poor concentration, poor memory, difficulty making decisions, insomnia, nightmares, weakness, difficulty working under pressure, heavy breathing, intense hunger, always need to nibble, dizziness, blurred vision, hallucinations, convulsion/seizures, heart palpitations, excessive or unexplained sweating
  • CNS symptoms occur if glucose drops gradually -headache, confusion, visual disturbances, changes in personality, convulsions, coma (assoc. with low body temperature)
  • Adrenergic symptoms when there is an acute drop in glucose (insulin reactions) – sweating, anxiety, dizziness, diaphoresis, nervousness, palpitations, s/t hunger (assoc. with acute, less marked decreases in plasma glucose.

Diet

  • Nuts and seeds for protein; especially almonds since they are easily assimilated
  • Two protein meals, one small starch meal daily: two to four snakes in between.food chich grill
  • On rising: medium orange, half grapefruit or juice
  • Breakfast: Fruit, one egg (two slices of turkey/ham/bacon optional), one slice of bread/toast, buttered, soymilk, decaf. coffee or weak tea.
  • Decrease sugar, caffeine and ETOH
  • Don’t eat fruit at all except for limes, lemons, grapefruit
  • Eat high Complex CHO.
  • Add sweet spices to foods (cinnamon, cloves); add lemon to H2O
  • Refined sugar causes factor released in gut which release insulin regardless of amount of sugar.

Supplements

  • Chromium increases tissue sensitivity to glucose, 500 mg 3 times a day
  • Niacinamide relieves symptoms of reactive hypoglycemia
  • Glutamine 1000mg 3 times a day
  • Whole foods nutritional supplements _ call out office


Physical Medicine

  •  Exercise improves insulin sensitivity
  • Get 20-40 minutes in 3 to 5 times a week
